May 20, 2014 Both full and part time positions Duties:
1.
Serve as Team Facilitator for clients served.
3.
Assume leadership responsibilities for day:
to:
day vocational service delivery.
Represent the philosophy of Hope Haven through decision:
making process.
5.
Review service vocational objective plans on a monthly basis.
Ensure required documentation is complete, accurate, and meets applicable standards.
6.
Facilitate the completion and review of vocational progress reports and ongoing vocational assessment based upon funder and/or service regulations with each client.
8.
Arrange and manage clients vocational funding and assure documentation of services are accurate and support attendance records and billing data.
9.
Accountable for supporting clients in applying for and maintaining benefits for which they are eligible (i.
e.
Social Security Disability, Title XIX, SSI).
11.
Maintain client records including orientation, transfer, and discharge reports for employment services.
12.
Coordinate job placement and support services in conjunction with Community Employment Coordinator, other Employment Specialists and members of the team (client, referring agents, significant family members and agency personnel.
) 13.
Ensure Outcome Measurement System data pertaining to employment services on each client is accurate and up to date.
14.
Arrange for implementation of needed job accommodations for clients as necessary.
16.
Adherence to Hope Haven Policies and Procedures.
Maintain all documentation required by agency policy and procedures.
18.
Other duties as assigned.
Qualifications:
1.
BA in Human Service related field and prefer two years experience in a social work related field or one year of case management experience in working with persons with developmental and/or psychiatric disabilities.
2.
Strong leadership ability.
3.
Ability knowledge of mental, physical, and social impairments and their probable effects on vocational, personal, social, and community adjustment.
4.
Skill in Behavior Management.
5.
Excellent oral and written communication skills.
7.
Skills in computer use and word processing.
8.
Knowledge of and ability to utilize assistive technology and adaptive equipment.
9.
Possess Class D chauffeurs license with endorsement 3.
10.
Negative drug screening.
11.
Up to date physical.
12.
Clearance from a criminal background check and abuse reporting check at time of hire.
